Why I'm Building ElinMed

I spent years practicing inside a system that made it genuinely difficult to practice good medicine. Insurance requirements determined which tests were approved and which weren't. Appointment slots were short by design, not by accident. Patients with complex cardiac histories were evaluated in 15 minutes, and the most important conversations — about risk, about prevention, about what they actually wanted from their care — rarely happened at all.

ElinMed is my answer to that. A practice where I can give every patient the time their situation requires, review their records before they arrive, reach them between visits when something changes, and build a clinical relationship that develops meaningfully over time. Where decisions are made based on medicine — not billing codes, prior authorizations, or referral patterns.

It is not a complicated idea. It is simply the practice I always wanted to run.

A Prevention-First Practice

The dominant model of cardiology is organized around acute events — a heart attack, a hospitalization, a procedure. Preventive care exists within that system, but it tends to be squeezed into what remains after the acute cases are managed.

ElinMed inverts that structure. The focus is on understanding a patient's cardiovascular risk trajectory over time, identifying meaningful changes before they become clinical events, and having the time and relationship to act on those findings. A new patient visit is 60 minutes because it takes that long to do the job properly. Advanced tools — detailed biomarker panels, specialized cardiac imaging — are incorporated when they're clinically appropriate. They inform decision-making; they don't define the practice.

Vision for the Practice

The long-term goal for ElinMed is a practice where preventive cardiovascular care is genuinely longitudinal — where patients don't arrive in crisis but rather maintain an ongoing relationship with a physician who tracks their cardiac health over years and decades, and who is reachable when something changes. ElinMed accepts virtual patients across New York State now, with in-person care at Midtown East launching Fall 2026, and planned expansion to Georgia, Florida, and Texas.

How the Practice Works

A direct physician relationship,
without intermediaries.

ElinMed operates outside the insurance billing system — not as a statement against insurance, but because direct-pay practice allows for a fundamentally different kind of care.

Appointments structured by clinical need

When Dr. Nyange schedules a new patient visit for 60 minutes, it is because 60 minutes is what the evaluation requires. Follow-up length reflects the complexity of the conversation, not a billing tier.

Direct access to the physician

Questions and concerns between visits reach Dr. Nyange via our secure portal. This provides meaningful, ongoing continuity for patients—with messaging frequencies calibrated depending on your active membership level.

Fees stated in advance

Every visit fee is published and fixed. A virtual new patient consultation is $400. An in-person Comprehensive Cardiovascular Evaluation is $750. No facility fees, no level-of-service adjustments, no surprises.

Testing ordered on clinical grounds

Labs, imaging, and specialized cardiac assessments are recommended because they are clinically appropriate for a specific patient — not because they fall within a pre-approved list.

Insurance & Reimbursement

ElinMed does not contract directly with insurance providers. A detailed superbill is provided after each visit for patients who wish to submit for potential out-of-network reimbursement. Coverage varies by plan — patients with PPO plans may be able to recover a meaningful portion of the visit fee.
